The only 'technical' issue I am aware of is VIO distinction...and I believe 
that could easily be done with a jumper, barring problems finding a 
'universal' connector for the top of the extender.  I am sure you could 
easily have one made or machined from an available connector.

Well, not necessarily.  Some manufacturers didn't (follow the spec and) 
make the two connector PCB cavity to end dimensions the same...so 64 bit 
cards don't plug in correctly when these connectors are turned around!  The 
64 bit cards have to have the front of the rear connector shaved to fit...
