FYI, I have come across a V I/O problem during system-integration with PCI riser cards:
VI/O is electrically tied to 5V on the extender we received. (The keying for the motherboard end is universal, while the keying for the card-insertion end is 5V.) Thus trying to use this extender with a universal card, and a 3.3V motherboard becomes a problem.
The solution is to match the extender (which must have either a 3.3V or 5V card-end connector) with the motherboard. The supplier of the 1U system we bought did not pay attention to this detail.
