Don't be confused with the test case.
The data hand-shaking conditions for PCI transaction is very simple:
During a transaction, when both TRDY# and IRDY# are asserted, data is
transferred.
The test case is to describe a situation to see if your design works for
the special situation.

Then, Item 2 would mean
clk 1: TRDY# asserted
clk 2: TRDY# deasserted ( aka released ), wait state.
clk 3: TRDY# asserted
This initial TRDY assertion is not allowed for read operation because
the cycle is a turn-around cycle, is it?
